Additional fields in message templates

All Message Templates should have the ability to have more fields to be filled from the profile.

Specifically for Observation Checklists or Supervisor reviews, the LEARNER name, shift, department, job title, etc should be available to help the Reviewer or Supervisor know who needs review or follow up.
