Add Comments field when marking a training complete

There should be a comment or notes field to enter details when marking a course complete. It's important to our organization as we are heavily regulated.

    Our company is regulated by the FDA. We sometimes do group training on Standard Operating Proceures that would normally require an electronic signature. I receive a printed report with the employee's signature, which I then use to mark the enrolment complete. As 12 CFR Part 11 prevents us from modifying an e-signature, the only explanation I can provide an auditor is "the completion was likely recorded on paper". It would be nice to document that in a notes field so (in my absence) another System Admin would know the find the scanned record in SharePoint.
