adding other as a custom field

It would be great if an admin can create a custom field for registration where one of the drop down selections is "other." If the user selects "other," the user will be prompted to define "other" in a open text field.
We would like the ability to edit the names on course fields (ex. goals, audience, etc) and/or the ability to add custom ields at the course level (can add them for user but not course).
