To whom it may concern. Please consider these suggestion to upgrage the external training form :
1) Have the ability to enter a starting and ending dates, for training done on multiple days.
2) For the "Total time spent (hours), remove the arrow (on the user side), to avoid decimals (e.g.: 0.19), but only keep 30 minute increments.
3) Have the ability to customize the label name
4) Have a field to add notes
5) Have the ability to add a short description on how to use the field properly
6) Have the ability, from the user experience, to have access to all submitted request, their status and their attachments.
